When you arrive at the Fox Chase main campus, please go to New Patient Registration in the West Building. We ask that you come 45 minutes early if you have not pre-registered online through your myTempleHealth account (formerly myFoxChase).
If you are getting radiation treatment, please go directly to the Department of Radiation Oncology on the lower level of the Young Pavilion.
If you need to go to our Huntingdon Pike or Buckingham locations, you will be told how to get there when you make your appointment.
For help finding your way during your visit, please visit Patient Support Services desk on Level 1 of the West Building.
During registration you will be asked to sign the following forms:
- General consent – This form gives Fox Chase Cancer Center, our doctors and nurses approval to read your medical records, decide what your medical needs are and give you care.
- HIPAA – This form tells you about your right to privacy.
