Fox Chase Medical Records Department is Moving to Friends Hall at Temple Health - Jeanes Campus

Effective Tuesday, March 22, 2022, the Fox Chase Medical Records Department is moving to a new location and will start a new process for patients who want a copy of their films or a copy of their medical records. Patients requesting a copy of their medical records should go to Friends Hall at Temple Health - Jeanes Campus. Patients requesting aa copy of their films should go to Diagnostic Imaging on the 1st Floor of the Main Hospital.

As a patient of Fox Chase, or as a family member or guardian of a patient at this hospital, we want to inform you of your rights under federal and Pennsylvania state law. We are committed to honoring your rights. You and your family also have certain responsibilities to help us provide the best possible care. A copy of complete patient rights and responsibilities is available upon request.

Policies Relating to HIPAA, Patient Privacy, and Information Security

We take the privacy and security of our patients and their protected health information seriously. Fox Chase is subject to regulation under the federal Health Insurance Portability and Accountability Act (HIPAA), which, among other things, protects the privacy and confidentiality of patient information. Our goal is to ensure that patient privacy and confidentiality is a top priority, and all employees are aware of their role in regard to the proper handling of protected health information.

Fox Chase provides a confidential hotline for patients and staff to put you directly in touch with our Office of Compliance and Privacy through Temple University Health System (TUHS). You may choose to leave your message anonymously, but please give enough detail so we may address your concern.

Patient Test Result Information Act

Act 112 – known as the Patient Test Result Information Act –is a Pennsylvania law effective in 2019. Based on what is required by the law, if Fox Chase patients have a diagnostic imaging test (CT, MRI, PET) performed that shows an unanticipated significant abnormality, patients will either receive that information directly from their healthcare provider (e.g. treating clinician) before leaving or will have a letter with that information sent to their address through the United States Postal Service. This is to provide every opportunity for patients to have a discussion with their providers about the result.

Non-Discrimination Policy

It is the policy of Fox Chase Cancer Center that there shall be no exclusion from, or participation in, and no one denied the benefits of, the delivery of quality medical care on the basis of race, ethnicity, religion, sexual orientation, gender, gender identity/expression, disability, age, ancestry, color, national origin, physical ability, level of education, or source of payment.

Pennsylvania Wiretapping Law

It is a violation of Fox Chase policy and state confidentiality laws to use any device to record conversations or telephone calls of patients or staff without consent of all parties. This includes cell phones, cameras or any other electronic devices.

How to Request a Copy of Your Medical Records

Requests for your medical records must be made in writing. To complete your request, we may charge a fee for costs of copying, mailing or other supplies.

How to Request a Copy of Your Films

Diagnostic Imaging: 215-728-3879
Patients should contact diagnostic imaging staff to request a CD copy of their films.
