My philosophy of care is centered on compassion, patient advocacy, and individualized support. As a nurse navigator, I focus on understanding each patient’s unique needs, coordinating appropriate resources, and ensuring they are prepared for a safe and successful transition home. I value clear communication, collaboration with the interdisciplinary team, respect for patient preferences and autonomy, and addressing barriers to care before discharge. My goal is to help patients and families feel informed, supported, and confident in managing their care after leaving the hospital.
